New Patients: For the first visit you will have to fill out a New Patient Packet prior to your child's visit with the doctor. To skip the wait in filling this out, you can click
here to download and print and fill out this packet before the visit so you can be seen sooner.
Don't forget to bring your insurance information with you
In order to register and treat you promptly, we request that you bring your child's current insurance information with you for each visit. For Well Child visits we ask that parents not only bring
their child's current insurance but also their shot record so that we may keep it up to date.

We accept many more insurances, if your insurance is not on the list please feel free to ask one of our helpful receptionists if we accept a certain insurance.
Co-payments are collected at the time of the visit. Due to our contract with your insurance, we are legally required to collect them at the time of the visit. If there are any services that were
not covered by your insurance and put as your responsibility it will be collected at the time of your next visit. However, if it is a large amount, please feel to contact us to set up a payment plan
with our billing department. We are here to work with you if you are unable to pay the full balance in full.
